SAFESPORT OVERVIEW
WYHA is committed to creating a safe and positive environment for its participant’s physical, emotional, and social development and to ensure that it promotes an environment free from abuse and misconduct. As part of this commitment WYHA will be following the recommendations and guidelines defined in the USA Hockey SafeSport Program Handbook. Any questions or concerns should be directed to WYHA’s SafeSport Coordinator at safesport@wayzatahockey.org.

SafeSport and Background Checks Overview
All Coaches, Team Managers, Locker Room Monitors, Board Members, Coordinators, and any other volunteer that will have regular contact with or authority over WYHA players are required to complete the following registration steps. Follow the steps on the Volunteering page to complete your requirements. It is strongly recommended to get a head start on these requirements early in the season or pre-season to ensure you are compliant. Lack of compliance will result in being unable to do the role (ex. a Coach will not be allowed on the ice).
SafeSport Reporting
In the event that any WYHA member observes any SafeSport abuses and/or misconduct, it is the member’s personal responsibility to immediately report their observations to the WYHA’s SafeSport Coordinator. Suspected child physical or sexual abuse should be reported to appropriate law enforcement authorities.
WYHA has ZERO TOLERANCE for abuse and misconduct.
To report a SafeSport Incident/Violation, you can use any of these reporting options:
- Fill out the WYHA Code of Conduct/SafeSport Incident Report Form
- Email the WYHA SafeSport Coordinator at safesport@wayzatahockey.org
- Report the incident directly to USA Hockey SafeSport safesport@usahockey.org or 1-800-888-4656
Code of Conduct and/or SafeSport Issue Reporting Form
A Code of Conduct and/or SafeSport issue can be reported via the form below. The form will route to the WYHA President, VP of Administration, VP of Travel, and VP of Girls Travel.
USA Hockey SafeSport and Background Check Confirmation Report
Please see the chart below for SafeSport and Background Screen completion and expiration status. You must register as a WYHA Volunteer to check your status. Please note, the WYHA Registrar cannot resolve any SafeSport or Background Screening concerns. Please contact USAH at memberservices@usahockey.com, please include your current USAH number in your email.
