CODE OF CONDUCT FOR PLAYERS AND PARENTS/GUARDIANS
Agreeing to the WYHA Player & Parent/Guardian Code of Conduct is part of the registration process for each player and their parents/guardians. All players and parents/guardians are expected to know the code of conduct. If necessary, please reference this document for managing problems or concerns that arise. Thank you.

The following form exists as a means to share Code of Conduct or Inclusion & Diversity concerns, should you feel the need to escalate an issue. It will route to a committee within the Board of Directors who will follow-up with you.
