Volunteer Requirements Tracker
The LRM and Team Manager Registration Form serves as the official method for indicating the team on which you are volunteering with. Completion of this form is required in order for WYHA to accurately track compliance with all rostering requirements and to ensure you are placed on the correct team roster.
Individuals who previously registered for the purpose of tracking requirements are thanked for doing so; however, submission of this form is still required to designate the specific team to which you are to be assigned.
Please note that requests submitted via email will not be accepted — all assignments must be processed through this registration form.
Only teams with rostered players will appear in the team selection list. Prior registrations remain valuable, as they allowed you to begin fulfilling your requirements and provided a record of your progress.
Registering enables your name to pull into the tracking report here, so that you can clearly see if your SafeSport and Background Check are up to date.
There are two reasons to use this registration.
- You intend to be a LRM for your players Travel team, or a Team Manager for your players Travel or Mite team.
- A parent/guardian of a Travel or Mite player who intends to sign-up for LRM Association Volunteering Shift posted on the Volunteering page (usually during Tryouts and for Goalie Skills). Registering here does NOT automatically sign you up for these shifts. You will need to sign-up for specific Shifts on the Volunteering page. See additional directions on that here.

WYHA 2026-2027 Tracking
This registration is designed to help WYHA track the completion of requirements for parents interested in serving as Team Managers or Locker Room Monitors. Completing this registration allows us to identify where you are in the requirements process and communicate any outstanding items that must be completed before you are eligible to be rostered.
Once players have been assigned to their teams, all Team Managers and Locker Room Monitors will be required to complete a separate team-specific registration in order to be added to the official USA Hockey roster. This team-specific registration is required for everyone serving in these roles, with no exceptions.
